Kidney Foundation of Medina County The Kidney Foundation of Medina Countys Educational Program is designed to assist individuals with kidney disease(or their spouse) gain skills necessary for employment. Who is Eligible? Any kidney patient residing in Medina County. Spouses of a Medina County kidney patient are also eligible. How Do I Apply? Completed applications must be mailed to the Foundation. Facsimile applications will not be accepted. Incomplete applications will not be considered for funding. Decisions regarding funding will be made by the Educational Committee, and applicants will be notified by mail. Individuals may be eligible to apply each year until completion of their education. How Does It Work? Priority will be given to patients with kidney disease. Spouses will be considered next. Payment will be made directly to the educational institution. Students are expected to show proof of completion of the course of study for which assistance has been received from the Kidney Foundation of Medina County. Recipients may be asked to reimburse the Foundation for dropped classes. Renewal applications will not be considered from individuals who have misused Foundation education funds. The decision of the Educational Committee is final. Individuals are encouraged to apply each year they are eligible, though there is no guarantee of continued funding. A grade of C or higher (or equivalent) must be maintained to be considered for multi-year funding. In order to re-apply for funding, an original transcript of grade(s) or a report card must be submitted with the application (these will be returned to you).
